This membership plan is designed to provide affordability and greater access to quality dental care. This plan is intended for the use of families living in the same household. This is a discount plan, not dental insurance.
WHAT TO EXPECT:

Immediate eligibility (no waiting periods)
If a plan member has been diagnosed with Periodontal Disease and requires 4 cleanings/year, an additional $10/month will be added to the monthly price.

Coverage

2 Regular Cleanings per year

2 Fluoride Treatments per year

2 Exams per year

1 Emergency Exam per year (same day or after hours)

Annual X-rays (CBCTs not included. CBCT is a 3D image used to diagnose things not seen on conventional 2D images-this may be needed when planning dental implant surgery.)
All additional treatments not mentioned above will be discounted at 15%.
Plan Cost
The plan agreement will last for a duration of 12 months. After the first 12 months, the agreement will then be made for a duration of 6 months.
If paying with a Debit Card/HSA/Flex Spending Account:

$25 per month for the first member and $20 per month for each additional family member.
If paying with a credit card:

$27 per month for the first member and $22 per month for each additional family member.
*If a plan member has been diagnosed with Periodontal Disease and requires 4 cleanings per year, an additional $10 per month will be added to the monthly price.
